Are you a passionate healthcare professional looking to make a meaningful impact in patients' lives? We are seeking a Part-Time Remote Care Coordinator to join our growing team! In this role, you'll deliver high-quality care coordination services to chronically ill patients, helping them manage their conditions, develop personalized care plans, and provide much-needed support between office visits.

What You'll Do:

Conduct monthly phone calls with patients, practices, and pharmacies to ensure patients are receiving necessary care.
Develop personalized care plans, collaborating with medical providers and staff to address the needs of each patient.
Identify gaps in care, social determinants of health, and other factors to help connect patients with available community resources.
Empower patients to take control of their wellness by offering education and guidance on managing their conditions.
Manage a high call volume daily, aiming to achieve your productivity goals and maintain patient-centered care.
Maintain patient confidentiality and ensure adherence to ethical, legal, and accreditation standards.
Who You Are:

You're a Certified or Registered Medical Assistant with at least 1 year of experience.
You're compassionate, organized, and driven to deliver excellent patient care.
You have experience with EMR/EHR systems and are comfortable using technology to provide care in a remote environment.
You thrive in a fast-paced, remote work setting and are self-motivated to meet productivity goals while delivering personalized care.
You have strong verbal communication and writing skills, allowing you to engage with patients and medical professionals effectively.
Preferred Experience:

Call center or triage experience.
Experience working with geriatric populations or in case management/care coordination.
Your Work Environment:
This is a part-time, fully remote position. You'll need a private home office space, reliable high-speed internet (upload speed of at least 50 Mbps), and a second monitor to efficiently manage your workload.
